Since your login ID serves as the email address that is used to contact you, you cannot change it.
It will be used and associated with an answered questionnaire permanently. You may use another email address
and sign in again as a new user. If you answer questionnaire(s) with an expired email address, please contact
your system administrator and request that the email address be changed, and include the email address that
should be used to replace it.

you cannot remember your password.

Click this radio button if you have never signed up before.

Enter an email address that you will always use when sending in questionnaires to us, and is good for receiving email information from us.

Select a 'Hint' question that you will remember.

Type in and remember the 'Hint Answer'. You should select a hint question and answer that you will easily remember. The hint answer must be
entered exactly as it was initially in order for this feature to properly operate.

Click the Sign In button.

the 'I've forgotten my password.' radio button.

Your 'Hint' question should appear selected in the dropdown list.

Type the hint answer exactly as you did when you initially setup your email login ID and password.

You may name a questionnaire session anything you'd like. As an exmaple, you may call the questionnaire something related to your
event, such as 'Boy Scouts'.

If at any time you need to walk away before clicking the final Finish button, you may click the Save and Logout button.

Then, the next time you log in, you should see 'Boy Scouts' under Existing Forms.

Click on the Edit link within the grid, and you should see the first question with all the previously entered information.

Click on the Next button until you find the first blank question, where you had previously left off.

Clicking the Next button will save the answer(s) on a given questionnaire page. It will then logically proceed in the
questionnaire process to the next question. The Next button will always appear on the screen, until the Finish
button is displayed on the final question.

Clicking the Back button will NOT save the answer(s) on the current questionnaire page. It will display the previously
answered question. This is useful if you noticed something was not entered correctly, but you hit Next by accident.

Clicking the Save and Logout button will immediately save the current question, and log you out of the questionnaire.
You should note the name that you called the questionnaire, as you will need to select it in order to resume answering the
questionnaire with all saved answered.

Clicking the Finish button will immediately save the LAST question, and log you out of the questionnaire.
The questionnaire is then forwarded to the administration staff for further review. A confirmation page should
display showing you a User Questionnaire #. Retain this # for future reference during communications with the
administrative staff.
